5th August, 2023   GM case -   Case scenario ..... Hi, I am S. Pranathi, a student  of 3rd BDS. This is an online elog book to discuss our patient health data after taking his consent.  This also reflects my patient centered online learning portfolio .                                           Case history Cheif complaint: A 670yr old male came with complaints of fever since 2 days and wound on the right foot History of present illness: Patient was apparently asymptomatic 2 years ago then he had a wound on the right foot it is not healed since 2 years even he visited so many hospitals  The pain in the right foot is flight pain,tingling and numbness present Fever since 2 days, intermittent type History of past illness: Diabetes and Hypertension diagnosed incidentally No similar complaint like epilepsy, asthma, CVA, T. This is an online eblog book to discuss our patients health data after taking his consent.This also reflects my patient centered online learning portfolio.                             CASE HISTORY Cheif complaint: A74 years old male from dislicgerla came with complainta of both legs swelling , chest pain, breathing since 2 months History of present illness: Patient was apparently asymptomatic since 3years back Both legs swelling extend upto mid calf from 2-3 years it is intermittent , pitting type edema  Chest pain since 3 months continues with radiation  Breathing since 2 months no aggressive or releaving factors , breathing progress to grade 2 to grade 3 History of past illness:  Diabetes since 15 years Hypertension since 10 years  Edema since 3 years No similar complaint like epilepsy, asthma, CVA, T. This is an online eblog book to discuss our patients health data after taking his consent.This also reflects my patient centered online learning portfolio.                             CASE HISTORY Cheif complaint: A 60yr old female came with complaints of fever since 10 days -productive cough and pain in left chest region while coughing since 10 days -vomitings since 10days  HOPI - Patient was aparently asymptomatic 10 days back and She then developed fever low grade,intermittent,not associated with chills and rigors,relieved on medications C/o cough with expectoration since 10 dayswhitish mucoid sputum not blood tinged C/o nausea and vomitings since 10days 2-3episodes per day which is watery,non projectile ,non blood tinged with food particles as contents C/o constipation and decreased appetite since10 days C/o pain in the back while coughing No H/O Burning micturition, loose stools, pain abdomen No h/o p

This is an online eblog book to discuss our patients health data after taking his consent.This also reflects my patient centered online learning portfolio.                            CASE HISTORY Patient details  A 21 years old male Resident of choutuppal ,occupation by a  student  Chief complaint : Pain near the chest region followed by severe pain near the abdominal region since  2 week. Multiple episodes[30-40 times] of vomiting and breathlessness Since 2 days . History of present illness  Patient was apparently asymptomatic 2 weeks back then had pain near the chest region and  followed by pain near the epigastric and umbilicus region which is insidious in onset and gradually progressive.....pain type dull thrombing . And patient is having multiple episodes of vomiting, that is 30-40 times  per day since 2 days before coming to hospital.... Vomiting characteristics : 1)Non-projectile
